New Patients

Hello, and welcome new patients!

Below, you will find a link to our new patient form.

Please bring your completed form to your first appointment.

You will be asked to place a $50 deposit to hold your new patient appointment.

This deposit is to be considered non-refundable unless a minimum of 24 hour notice is given prior to changing or cancelling your first appointment.  This deposit may be used to pay for any medication, co-pays or other expenses incurred during your treatment.

For your first appointment, please bring a photo ID (such as a driver's license)  and insurance card(s).

Since this is a medical appointment, we also ask that you bring any recent lab testing or other tests which you may think could be helpful.  Ideally, this would include fasting blood sugar, cholesterol, triglyceride, testosterone for men and thyroid function tests from within the year.

We will attempt to submit to your insurance company(s), but it is your responsibility to understand what coverage your health insurance provides.  You will ultimately be responsible for your bill if insurance does not cover your appointments.

Any medications or meal replacement items will need to be paid for at the time of the appointment.  Any balances must be satisfied at each appointment and prior to obtaining  medications or other products from our office.
